Our approach to your support
At Personalised Disability Care, our NDIS care team talk to you and the people who are important to you. We really listen to your thoughts about how your life should be lived. Only then do we work with you, your family, community and health network to ensure the personalised support we offer is aligned with your goals.
NDIS care team always offer you full choice and control over which carers become part of your support team and how your care is delivered. Because "one size fits all" has no place in disability support services.
Our NDIS Services
Assistance with household tasks
Our team will assist with all the tasks and everyday activities that help ensure your home environment is a healthy, tidy space for you to live. Whether it's light housework, laundry, gardening or other domestic chores, our friendly support workers help you maintain a fresh and inviting living space.
Assistance with daily personal activities
There's nothing better than feeling like you're all ready for the day. Our support team will help with all those tasks that make you look and feel your best, including personal grooming and hygiene, showering, dressing, getting around the house and helping you maintain your mobility and independence at home.
Assistance with daily life tasks in a group or shared living arrangement
Our team will help you to thrive in your shared living environment (including short-term respite care), so that you can continue to enjoy the companionship of your housemates. We can assist you in completing everyday living activities or supervise and help you in developing your skills so you can live as independently as possible.
Assistance with travel / transport arrangements
Our team will help you get out and about in the community, visiting friends and family or attending appointments. We act as your trusted partner, providing taxis or specialist transport arrangements so you can get where you need to go in comfort. We can also support you in building your skills to travel independently on public transport.
Assistance with social and community participation
Life beyond your front door can be exciting and rewarding and we're here to ensure you get to enjoy every moment of it. We provide transport, companionship and personal support when you're enjoying community-based activities, visiting friends or family or learning something new.
Plan Management
We work with you to get the best value possible out of your NDIS plan. You retain total control over the service providers you choose to work with while we sort out the administration, process payments, take care of financial reporting and keep track of your funds. It's one less thing you need to worry about.
